Seeing it's near a past growing segment, I think it's some sort of 'old' and healed damage, could be a lot: cold, sunburn, beasties,...

I'm not so sure it's rot. I get something a lot like this on my Carnegieas and it appears to be associated with annular growth (they are both healthy plants).

Strange. I can't say for sure what that is. Perhaps there is a critter in there eating away? Insects almost always go for new growth. Since the plant is actively growing it shouldn't be a water problem unless you are absolutely drowning it. Sunburn would affect the upper surfaces more directionally, I would expect....

Similar thing happened to my Pachycereus. I think its normal, the spots came when its spikes and stem was growing and disappeared when it stopped growing.
