Out of curiosity: does this Trichocereus always get a pup out of an areole like this?

Most cacti produce pups from areoles. The exception is Echinocereus, which has offsets pop out through the skin of the cactus - but that's the only one I know of.

I think it is always from part of the areole, though in the example above and in Echinocereus it is part of the areole that is buried under the epidermis, therefore the flower or offset has to break through the skin rather as a human tooth emerges from the gum. In some cacti (Coryphantha etc) the areole is lengthened from the axil to the tip of the tubercle by a groove along the top of the tubercle and spines are born at the spiniferous tip of the tubercle but flowers at the axil:-
