Hi
I've been grafting Aztekium hintonii buds (from multiheaded, ancient grafted specimens) onto Pilosocereus. every now and then, one won't take. A couple sprouted roots almost immediately after being rejected from the Pilosocereus. Is this a regularly observed phenomenon? Or is it the luck of small number statistics?

I don't see how it would make a difference. Have you done a control where you take an offset and try to root it without attempting to graft it first?

The forming roots from a scion due to the grafting failure is quite common.
It will happen on any cactus species.
The grafting of an A. hintonii offset needs a special setup because the scion is hard.
I have never use Pilosocereus as grafting stocks but I have experienced by other stocks.
I do not know your grafting method but:
- the size of scion
- pressure on the scion
will affect on the result.
If you could use Pereskiopsis as the stock, you can graft an A. hintonii offset easily without any pressure on the scion.
It will happen on any cactus species.

It could be that the graft "feels" that there is humidity under it (from the Pilosocereus), and sends some roots out in search for water...?

It may get a hormonal boost from the stock before the union fails too. The rootstock can dramatically affect how the scion grows.
