but I DO know that it has stuck with me for years and deserves to get better care! Please tell me what this rugged little thing is and how I should be properly caring for it. It is becoming root bound and I'm not sure if I should transplant it.

looks like Rhipsalis cereuscula (so a jungle cactus)

Yes, it doesn't look much like what most people expect a cactus to be.

Anyway, if it's root bound you should repot it. Not in the kind of soil used for desert cacti - epiphytes such as this like a more organic soil, in their natural environment they grow in mostly leaf mold.
