How long have you had the plant, and what type of soil is it in? In a low-light indoor situation, I'd be worried about rot. I second the notion that you should pull it out of the soil and inspect the roots.

They are looking good!! Looks like you'll have quite a few nice cacti there once they grow up. I can totally sympathize with your lack of sunshine - I actually have a few plants hanging between a window and a fluorescent grow light. They grow toward the fluorescent light haha!

I would probably try and avoid breaking any spines by placing it on a cushioned surface, perhaps an large folded towel or similar material. I'm certainly no expert, but plants in general don't have the regenerative abilities of animals, and I believe any damage to the spines would just callous over ...

This is just my experience, but I've found perlite to work perfectly fine provided I top dress with a good layer of gravel. That keeps everything from floating and I wouldn't be without it.
