That is one of the grafted gymnocalcium varieties. They tend to die off after awhile, which is why you should always be grafting the offshoots onto new plants. I think your best bet would be to cut the orange part (the gymno) off the stem and regraft it. If you don't know how to graft, there's plent...

Not to my knowledge. As far as I know, blue is one of the few colours cacti can't do.

Unless I miss my guess, that's Optunia microdasys. It looks healthy enough, it just needs more light. I suggest putting it by a sunny window. Just a word of warning: don't touch it if at all possible. Optunias leave horrid little spines in your skin if you touch them. Handle the plant with newspaper...
